Health Benefits

Overview

麻豆原创 offers a range of health benefit options designed to support employees and their families. Coverage includes medical, dental, vision, and additional voluntary benefits.

Enrollment & Eligibility

Health coverage begins on the first day of the month coinciding with, or following, your hire date. If your position starts after the first day of the month, coverage will begin on the first day of the following month. Employees must enroll in benefits within 15 days of their start date.

Eligibility varies by employee type:

Academic & Administrative Faculty

Benefits are available to academic faculty under 鈥淎鈥 (12-month) or 鈥淏鈥 (9-month) contracts, and administrative faculty working at least 50% full-time equivalent (FTE).

Classified Staff

Classified staff employees must work at least 80 hours per month to be eligible for benefits.

Letter of Appointment

Benefits are provided to eligible LOA employees who are assigned exempt duties under federal FLSA regulations and who work at 50% FTE or more for a period of less than twenty four months. Eligibility may also apply to Part Time Instructors depending on the number of credits taught each semester.

Dependents

When enrolling in your health plan, you may also cover eligible family members and dependents, including:

  • Spouse – Your lawful spouse under state law
  • Domestic Partner – Registered with the 麻豆原创 Secretary of State
  • Children/Stepchildren – Dependents 26 or younger, including stepchildren, domestic partner鈥檚 children, legally adopted children, or children under your legal guardianship who live with you. Coverage ends at the end of the month in which the child turns 26.

Making Changes

You may add or remove dependents during open enrollment in May or within 60 days of a qualified life event (QLE) via the PEBP .

Common Qualified Life Events (QLE)

  • Marriage, divorce, or domestic partnership changes
  • Birth, adoption, or legal guardianship of a child
  • Death or disability of a dependent
  • Loss or gain of other coverage (e.g., spouse鈥檚 employer, Medicare)
  • Change of residence affecting health network coverage
  • Receipt of a qualified medical child support order

Coverage Start & End

Coverage for you and your dependents begins on the first day of the month coinciding with or following your hire date and ends on the last day of the month in which employment terminates. Coverage is continuous as long as you remain employed and enrolled.

Plan Options

麻豆原创 offers three options:

  • Consumer Driven Health Plan (CDHP PP)
  • Low Deductible Plan (LD PPO)
  • Health Plan of 麻豆原创 (HMO)

Depending on your selected plan, your coverage may include:

Prescription Benefits

Prescription coverage varies by the medical plan you select. The Statewide CDHP (PPO and Low Deductible PPO) uses Express Scripts, while the HMO plan uses Optum Rx for prescription management.

Dental Benefits

Dental coverage is consistent across all medical plans. Coinsurance rates vary based on whether services are received in-network or out-of-network.

Vision Benefits

Vision coverage, copays, and benefits vary by plan. Most plans include coverage for glasses or contacts every two years, with an optional Visionary benefit available for added coverage.

Voluntary Benefits

Benefits-eligible employees may enroll in voluntary benefit programs. Participation does not require enrollment in NSU鈥檚 PEBP health insurance plans.

Medicare Plans

Medicare eligibility begins at age 65. Enrollment is not required if you are covered under a group medical plan through PEBP.

Health Savings Account & Health Reimbursement Arrangements

Employees enrolled in the PPO Consumer Driven Health Plan (CDHP) may qualify for an HSA or HRA to help cover eligible medical expenses with tax-advantaged savings.
